SMG is seeking a highly skilled and dedicated Senior Patient Care Coordinator to support our Behavioral Health services!

This role is ideal for an individual who consistently brings steadiness to fastpaced environments, provides clarity in complex situations, and approaches every patient interaction with empathy and professionalism. As a key member of our care coordination team, you will play an essential role in guiding patients and families through the behavioral health care process, ensuring they feel supported, informed, and connected at every step.

This position is responsible for the overall coordination of seamless care for patients in a surgical and/or non-surgical setting. They will serve as a mentor and resource to Patient Care Coordinators by providing support, guidance and training as needed.

Required Qualifications:

• 2 years experience in healthcare setting with increased responsibilities or 1 year experience in Patient Care Coordinator type role.

Preferred Qualifications:

• Electronic Medical Records (EMR) experience.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
